By submitting a transcript request, you acknowledge and agree that the University may require you to provide a valid government-issued photo identification to verify your identity before processing your request. You further authorize the University, or its authorized service provider, to authenticate and verify the information contained on the identification presented, including through applicable state motor vehicle or driver’s license verification systems, where permitted by law.

Submission of any falsified, altered, fraudulent, or misused identification, or any materially false information in connection with a transcript request, may result in denial or cancellation of the request, refusal of future services, referral for disciplinary action if applicable, and/or referral to appropriate law enforcement or regulatory authorities for investigation and potential civil or criminal penalties under applicable law.
